Rationale for change

The 2016 American College of Physicians (ACP) guidelines recommend steroids as a first-line treatment for all patients because of their significantly lower cost and better safety profile. The 2020 American College of Rheumatology (ACR) guidelines recommend that individual factors (e.g., side effects, contraindications, patient preference) guide the decision of which of these three options to pick, except in the case of NPO patients, for whom parenteral or intraarticular glucocorticoids are preferred.
