Correct me if I'm wrong, but I thought that loop diuretics are reserved for decompensated heart failure only. This question stem does not make that clear.
I added an Extra since it was the easiest change. You could also remove "especially in combination with loop diuretics" if you wanted to emphasize that MRAs are useful for chronic, compensated HF. You could also change the question stem to "decompensated heart failure" as the answer but that changes the focus from "use MRAs in HF" to "use loop diuretics + MRAs for decompensated HF".
cid:1497732771764 already has treatment of decompensated heart failure requiring IV loop diuretics.
