- A positive hepatojugular reflux indicates impaired relaxation of the right ventricle and consequent inability to accommodate an increased venous return to the heart (right heart is unable to pump additional blood volume).
- The most common causes of this finding include restrictive cardiomyopathy, constrictive pericarditis, and right ventricular heart failure (e.g. due to myocardial infarction)
- In patients with IVC obstruction (e.g., Budd-Chiari syndrome), the hepatojugular reflux is characteristically absent
