Stable pneumothorax treatment depends on size

Small pneumothorax (apex-to-cupola distance < 3 cm):
- Low risk (no underlying lung disease): observation (repeat chest x-ray in 4-6 hours)
- Higher risk (underlying lung disease): observation or chest tube
Large pneumothorax (apex-to-cupola distance ≥ 3 cm): chest tube regardless of risk
Unstable pneumothorax is usually treated as a tension pneumo
Suspected tension pneumothorax: needle thoracostomy → tube thoracostomy (chest tube)
