What are the next steps in a child with suspected bacterial meningitis and NO alarm symptoms?
{{c1::Blood cultures}} → {{c1::lumbar puncture}} → {{c1::IV antibiotics}}
Extra
- Antibiotics sterilize the CSF, so we get CSF fluid first unless the child demonstrates alarm symptoms
- If alarm symptoms (e.g. hypotension, status epilepticus, focal neurologic deficits) are present, get blood cultures STAT and initiate antibiotics immediately afterward, then obtain neuroimaging and obtain LP if not contraindicated
- Vancomycin for resistant Strep pneumo, third-gen cephalosporin for most strep. pneumo or neisseria strains
