Rationale for change

Would love to see this card be brought to match other similar cards like (anki id: 1543233325724) having multiple clozes instead of one giant one is much easier to recall and helps with thinking through in a step-wise manner.

Also removed 'aminoglycoside' from the text field to make the card a little cleaner. AMBOSS only mentions gentamicin specifically as a Tx here so the aminoglycoside part I think is unecessary.
