- The goal is to allow the bile synthesized in the liver to directly route to the intestines (instead of being stored in the bile duct, where it cannot escape)
- In this surgery: you expose the porta hepatis, excise the fibrotic extrahepatic bile ducts, and anastomose a Roux-en-Y limb of jejunum to the porta hepatis to allow bile drainage from the intrahepatic ducts
- The answer to this card used to be “distal duodenum,” but this has been corrected to “jejunum” to accurately describe the Kasai portoenterostomy (Dec 2025)
