- Often presents with painless otorrhea
- May be described as a white pearly mass behind the tympanic membrane
- Treatment is surgical excision
- Primary cholesteatoma: due to chronic negative pressure in the middle ear, causing retraction pockets in the tympanic membrane that become cystic and accumulate squamous cell debris
- Secondary cholesteatoma: due to tympanic membrane perforation (otitis media) allowing migration of squamous epithelium into the middle ear
- This card used to say "desquamated keratin debris," but that wording is not accurate for all types of cholesteatomas, and UW/AMBOSS/FA23 now use broader language (Jan 2024)
