Elevated TSH makes it unlikely that the nodule is functional (as the excess thyroid hormone would lead to feedback inhibition).

The Tx algorithm picture in the extra field and Amboss confirm that next best step in these patients should be fine needle aspiration

- *Nodule > 1 cm + high-risk feature on ultrasound → FNA

- *Nodule > 2 cm → FNA

- High risk sonographic features include irregular margins, microcalcifications, and/or internal vascularity

- If low TSH → thyroid scintigraphy/nuclear scan (often combined with RAIU scan)
