Rationale for change

I'm not sure if I'm right with this, but wont giving a strong vasodilator like Nitroglycerin cause "coronary steal" and actually acutely increase ischemia/cell death (possibly precipitating an NSTEM)? Could then something like a tPA/cath lab be used?
