This does not make sense to me. An adult that is not sexually active, why screen them for HIV? There is no good rationale in the Extra Notes either.

- Plasma HIV RNA testing is recommended for individuals with negative serologic tests and high clinical suspicion of acute HIV; testing for HSV, gonorrhea, syphilis, etc. is not needed in this sexually inactive, asymptomatic patient
- The recommended algorithm involves using a 4th generation assay to detect HIV-1/2 antigen/antibody combination along with checking for p24 antigen (from HIV-1).
- If all are negative, you're done. If any are positive, then proceed to antibody differentiation (to determine HIV-1 vs. 2) which can determine if HIV-1 or 2 antibodies are present. If indeterminate/negative, this is followed by PCR RNA for HIV-1 and 2
