- Either excessive bilirubin is being produced (haemolysis, etc.) or liver bilirubin metabolism is impaired (e.g. rifampicin jaundice).
- Consider choledocholithiasis as an alternative explanation for jaundice in patients with chronic hemolysis, since they have a high incidence of pigmented gallstones
