This card used to say quinine, but AMBOSS confirms quinidine is used for severe P. falciparum malaria (Sept 2023)
- Quinidine is used in the United States, quinine is used elsewhere
- Quinidine is a class 1A anti-arrhythmic drug that can target the erythrocytic stage of Plasmodium species
- Quinine works similarly to chloroquine, by inhibiting heme polymerization causing lysis of cell membranes
- Artesunate generates ROS which inhibits synthesis of proteins and nucleic acids
- If artesunate is not immediately available, begin interim oral treatment (e.g., artemether-lumefantrine, atovaquone-proguanil, quinidine, or mefloquine)
- Discontinue oral agent and administer IV artesunate as soon as it becomes available
