Rationale for change

There are four phases of the Valsalva maneuver. The reflex bradycardia of phase IV was previously mentioned in the Extra field but not in the main answer text. Furthermore, the mechanism for the HR decrease in phase I was not explained, nor were the phases explicitly spelled out.
