M-CENTOR is the standard in clinical practice. Can use M-CENTOR on anyone older than 3, and you will likely be asked to calculate score on peds rotation. Having this card focused on CENTOR and say it can only be used adults is an easy way to confuse students.

- In children, CENTOR is unreliable; must get a (+) rapid or culture before starting treatment!
Interpretation:
Score ≤ 1: no further diagnostic testing needed
Score 2-3: Consider rapid strep test and/or throat culture.
Score ≥ 4: Consider empiric antibiotic therapy (controversial)
- A rapid antigen test cannot be used to distinguish between GAS infection and carriers. Thus, the judicious use of a rapid antigen test for patients with a score ≥ 2 prevents the unnecessary treatment of GAS carriers.
- Even with a CENTOR score ≥ 4, empiric antibiotics without confirmatory testing are not routinely recommended as the risk of acute rheumatic fever is not high enough compared to risk of inappropriate antibiotic therapy
