Rationale for change

The previous cloze "proximal {{c1::esophageal atresia}} and distal {{c1::tracheoesphageal fistula}}" has little recall value. This asks the same thing but tests knowledge/recall better. Additionally "proximal" esophageal atresia is imprecise. It's just commonly just called esophageal atresia.
