- Clomiphene helps kickstart ovulation by antagonizing negative feedback ER receptors at the hypothalamus that regulate GnRH secretion → increased GnRH secretion → permissiveness of LH surge (and thus estrogen surge) to induce ovulation

- However, with regard to PCOS: Letrozole is an aromatase inhibitor that blocks conversion of androgen to estrogen, regulating LH and FSH levels and promoting the LH surge for ovulation

- Letrozole is preferred over clomiphene due to higher live birth rates (per UWorld)
