In addition to history and physical, patients initially diagnosed with hypertension should have the following basic testing, which may be remembered with the mnemonic HELP CUT blood pressure:
{{c1::HbA1c/Fasting glucose::H}}
{{c1::ECG (baseline)::E}}
{{c1::Lipid Profile::LP}}
{{c1::Chemistry panel::C}}
{{c1::Urinalysis::U}}
{{c1::TSH::T}}
{{c1::ECG (baseline)::E}}
{{c1::Lipid Profile::LP}}
{{c1::Chemistry panel::C}}
{{c1::Urinalysis::U}}
{{c1::TSH::T}}
Extra
- Urinalysis detects kidney damage (hematuria & urine protein/creatinine ratio)
- Chemistry panel (electrolytes/creatinine assessing for renal function, HbA1c assessing for diabetes)
- Lipid profile to screen for other atherosclerotic risk factors
- ECG to evaluate for CAD and LVH; look for changes associated with hypertensive heart disease
-TSH anomalies are one of the most missed causes of secondary hypertension (both hypo and hyperthyroidism can cause hypertension)
Initial evaluation of hypertension | |
Renal | - Serum electrolytes (Na, K, Ca) - Serum creatinine - Urinalysis - Urine albumin/creatinine ratio (optional) |
---|---|
Endocrine | - Fasting glucose or hemoglobin A1c - Lipid profile - TSH |
Cardiac | - ECG - Echocardiography (optional) |
Other | - CBC - Uric acid (optional) |
