- Persistent status epilepticus defined as continued seizure activity 20-40 minutes after onset
- IV phenytoin/fosphenytoin, levetiracetam, or valproic acid → IV phenobarbital → general anesthesia with IV midazolam, propofol or barbiturates
- Note: any of these same drugs may also be used after resolution of status epilepticus to prevent seizure recurrence
