Rationale for change
While alpha tetramers can form, the principle cause for extravascular hemolysis and ineffective erythropoiesis is due to the unpaired alpha chains precipitating together. These are still in a monomeric form, just clumped up togther.
"Unpaired α chains precipitate and damage RBC membrane, resulting in ineffective erythropoiesis and extravascular hemolysis (removal of circulating RBCs by the spleen)." - Pathoma
