Rationale for change

Several improvements for a card that ive always didn't like (odd formatting and structure with minimal context)

- expanded on what IQ means and added definition directly from AMBOSS addon (minor rewording). It now picks it up as well

- No need to repeat intellectual disability 3 times, so kept it a the top

- Moved the numbers to the right, feels weird having them on the left

- Improved the explanations in the extra which are unnecessarily hard to understand (could still improve more)
