2,3 BPG is an isomer of an intermediate in glycolysis (1,3-BPG) with two phosphate groups (note it's negative and when we exercise, a lot will be produced). So it can only bind to a hemoglobin without any of the four oxygen, it connects to two positive groups (remember 2,3 BPG is negative) and causes a conformational change that, when bound, causes hemoglobin to have a much lower affinity for oxygen.
