In card 1474686325582 (which I've suggested to delete) it only said heart failure. It's important to know/distinguish that ACEIs are only first-line for left side heart failure (specifically HFrEF aka systolic heart failure) as stated in both FA 2025 and Amboss in their heart failure article.
FA 2025 - Page 316 (Cardio > Path) states "ACE inhibitors, ARBs, angiotensin receptor–neprilysin inhibitors, beta-blockers (except in acute decompensated HF), and aldosterone receptor antagonists decrease mortality in HFrEF."

In the same article on AMBOSS the first line for HFpEF does not list ACEIs. It instead states "First-line agents
SGLT2i for all patients: e.g., dapagliflozin or empagliflozin
Loop diuretic for patients with congestion: e.g., furosemide or torsemide"
"systolic heart failure" is the term used in this card ID (1472487526769) I included the explanation from the extra section of that card to denote that systolic heart failure used to be synonymous with HFrEF.
Because reduced EF entails systolic and diastolic impairment, HFrEF is clearer and more accurate than systolic heart failure, which can mislead by overemphasizing systolic dysfunction.
Amboss states in the same heart failure article under Historical Terminology -- "Systolic HF has been used as a synonym for HF with reduced ejection fraction (HFrEF) but is no longer favored because individuals with HFrEF have both systolic and diastolic dysfunction."

This uWorld QID aligns with the Classification of HF by LVEF section in the Amboss Heart Failure Article it states in the HFrEF section -- "HF with reduced stroke volume and reduced LVEF (≤ 40%)"
