Age 2-24 months:
- 1st UTI → pelvic and renal US for anatomic abnormalities predisposing to vesicoureteral reflux → reassure if normal, don't need to perform VCUG
Age < 2 years with recurrent UTIs or UTI at age < 1 month or 1st UTI from organism other than E. coli or renal US showing renal scarring/hydronephrosis:
- Definitive diagnosis with VCUG
Other tests and their uses:
- Renal US: screen for hydronephrosis
- Renal scintigraphy with dimercaptosuccinic acid: evaluate for renal scarring (calyceal clubbing, focal parenchymal scarring) secondary to recurrent or chronic pyelonephritis
- Serial serum creatinine: renal function
- BP and CBC: chronic renal insufficiency leading to HTN, anemia
- Get an ultrasound after the first febrile UTI, but recurrent UTIs → voiding cystourethrogram
