- Dull abdominal pain that gradually worsens and localizes to the right abdomen, especially right upper quadrant (due to the gravid uterus), not to the right lower quadrant (McBurney point)
- Most common cause of acute abdomen in pregnancy
- Fever and fetal tachycardia may also be present
- Best initial imaging modality: abdominal ultrasound (for pregnant patients; non-pregnant patients → go for CT)
- Best next step: immediate surgery (appendectomy)
- Sterile pyuria is a common finding; can occur if part of the urinary tract (usually the ureters) is within close proximity of the inflamed appendix
- Uterine tenderness = chorioamnionitis
