Rationale for change

Both incarcerated hernia and strangulated hernias are indications for surgery - I kept answering strangulated for this question as it's the MOST urgent indication for surgery.

I think might be worthwhile to add a cloze deletion for strangulated as it's HY and been tested on UW and AMBOSS numerous times.
