Rationale for change

Following the same point made in a another suggestion (https://community.ankihub.net/t/05-24-2025-updated-content-anking-step-deck-ankingmed-id-3341537/472202?u=jwill)

TAH is extremely outdated. It is not the standard of care and is very rarely done anymore. TLH and even more so RALH, to be honest, are the right answer here.

In terms of Step exams, "Total hyst" should be sufficient without getting into the weeds of the technique.

AMBOSS supports "First-line: total hysterectomy with bilateral salpingo-oophorectomy
Laparoscopy is preferred."
