Rationale for change

Choice of the dmard depends on the underlying condition and patient factors.

Azathioprine/6-MP are preferred for GI/hepatic diseases (e.g., Crohn’s, autoimmune hepatitis) due to gut-specific efficacy.

Methotrexate is first-line for joint/skin diseases (e.g., RA, psoriasis) due to better evidence in these conditions.

A blanket statement favoring only azathioprine/6-MP could mislead learners treating non-GI diseases.

A rheumatologist would never choose azathioprine over MTX for RA.
