- Warfarin should NOT be used in the acute management of HIT (only transition to warfarin once the platelet count has recovered (> 150k))
- If HIT is suspected, all heparin products (including heparin flushes) should be discontinued immediately and for LIFE, and anticoagulation should be initiated with a nonheparin medication (e.g., argatroban, fondaparinux)
