For NBME purposes the best initial therapy is acetaminophen
"Ibuprofen is a reasonable alternative therapy for OA in patients who do not respond to acetaminophen. Acetaminophen is preferable for first-line treatment given the adverse effect profile of NSAIDs, which includes peptic ulcers and nephrotoxicity."
