Rationale for change

Changed the one-by-one to be on the name of the condition rather than the mnemonic - as the mnemonic is so visible, making it a cloze makes little sense.

I think if we were wanting a cloze for the mnemonic, it would need to be in a separate card (or including each letter in the cloze).
