Rationale for change
Heroin abuse is a well-documented secondary cause of focal segmental glomerulosclerosis (FSGS). Major nephrology sources, including UpToDate and Robbins Pathology, recognize this association. I missed answering obesity multiple times because I recalled heroin abuse as a cause. Adding it improves accuracy, aligns with Step 1 & Step 2 content, and enhances recall.
