Mycosis Fungoides Clinical Features (DermatoGraphix.com)
•Mic Fun Karaoke = Mycosis fungoides
•In a bathing suit = irregular erythematous scaly patches in non-sun-exposed/bathing suit distribution
•Itchy = may be pruritic
•Flat Rug = patch stage
•Raising microphone in the air = epidermotropic atypical lymphocytes (up high like the epidermis)
•Brain hat = cerebriform, hyperchromatic nuclei
•Pot of gold = Pautrier microabscesses
•Purple stripes on shirt = lined up at the DEJ with a band-like/lichenoid lymphocytic infiltrate
•Fibery wires on stage = wiry papillary dermal fibrosis
•Eating a lime = clue to epidermotropism is a lymph in every hole (intraepidermal lymphocytes out of proportion to the degree of spongiosis)
•Raised Chair = plaque stage
•Throwing microphone up in the air = more prominent epidermotropism
•Large booth = tumor stage
•Ripped seat = rapidly enlarging nodules with frequent ulceration
•Mic drop = decreased/absent epidermotropism
•Large quarter = large cell transformation defined by >25% large cells (>4 times the size of mature lymphocytes)
•CD from the 30s = CD30 expression may be present
•Sad child dropping CD from the 30s = CD30 negativity is associated with a poor prognosis
•Teams of 3 and 4 = typical phenotype is CD3+ and CD4+
•Groups of 7 and 8 not allowed = CD7 and sometimes CD 8 is negative
•Waitress with T menu ARRANGING a table = molecular testing for T-cell receptor gene rearrangement can be helpful in the patch stage
•Treatment of MF:
Treatments for patch/plaque stage (In the middle by rug and chair)
•Cooler = topical/intralesional steroids
•Mechanic holding mustard = topical nitrogen mustard/mechlorethamine
•Bricks = topical bexarotene/alitretinoin
•Camera = phototherapy
More advanced disease treatments (By the booth)
•Vortex = vorinostat
•Statue of ROMAN emperor = romidepsin
•Bent over in a tux = brentuximab
•MAGA hat = mogamulizumab
