I'm just not sure why intra-abdominal pressure would cause a tear. Intraabdominal pressure refers to peritoneal pressure, which would provide external compression on the esophagus.
Per amboss, it is purely pressure arising within the lumen of the GI tract (i.e. intraesophageal pressure) that causes a rupture
