Educational Objective: Felty syndrome should be suspected in patients with longstanding, poorly controlled, seropositive RA with new-onset splenomegaly and neutropenia. A peripheral smear should be evaluated to exclude LGL leukemia. Rituximab, a monoclonal antibody targeting CD20 expressed on the surface of B lymphocytes, is the treatment of choice.
