Rationale for change

Replacing acid reflux by GERD will better highlight the chronicity and long duration of CREST and the accompanying esophageal dismotility, while being more formal and in line with other cards (GERD is used way more frequently than simply acid reflux in the deck). Finally, Sketchy 1 mentions GERD.
