Per UpToDate algorithm "Nonpregnant, premenopausal patient with an adnexal mass on imaging":
A premeno patient who is high risk (with O-RADS 5, etc.) should undergo surgical management (cystectomy versus oophorectomy; depending on a number of factors such as CA-125 level, stage of disease, desire for future child-bearing which is complex here).
I did not find anything about biopsying ovarian cysts in premenopausal women in the literature. Correct me if I am wrong.

- Biopsy without cyst removal is contraindicated due to risk of seeding; cyst should be surgically removed → subsequent tissue biopsy
- May also be appropriate to obtain additional imaging (CT abdomen/pelvis, CT chest) to assess for metastatic spread if features of cyst are highly concerning for malignancy
