- Can't pee out phosphate, which ends up binding with Ca2+
- Secondary hyperparathyroidism is usually an adaptive response to prolonged hypocalcemia caused by either chronic kidney disease or vitamin D deficiency
- This condition results in parathyroid hyperplasia and excessive secretion of PTH, even though serum calcium levels may be low or normal
- Although uncommon, the condition may require surgical intervention, affecting only 1% of the US dialysis population each year
