Rationale for change
I dont really know how to change the table but copy pasted from answer
"endemic areas (eg, Ohio/Mississippi river valley). In the past, primary prophylaxis (eg, azithromycin) against mycobacterium avium complex (MAC) was given to those with CD4 counts ≤50/mm', but this is no longer recommended due to the low incidence of MAC (particularly once antiretroviral therapy is started)."

Rejection reason
According to your description of the question, the table is not in contradiction. The table indicates that azithro / clarithro can be used for prophylaxis in patients with "no active infection + not on fully suppressive ART"
