Rationale for change

This is part of a bulk suggestion to change some terminology and update cards about dehiscence.

Adding 'fascial dehiscence' here is helpful since this is the term used in Amboss add-on, and UW refers to this as 'deep (fascial) dehiscence

Amboss article:"Can be superficial (skin and subcutaneous tissue) or deep (fascial)"

Extra Extra
Text
{{c1::Superficial::Superficial / Deep}} dehiscence = intact rectus fascia and managed {{c2::conservatively (e.g., regular dressing changes)}}
