Rationale for change

This is part of a bulk suggestion to change some terminology and update cards about dehiscence.

Making it into two Clozes while maintaining the symmetry between the two lines can add extra focus on the initial step, which the Amboss qbank asks directly about.

The surgery for deep dehiscence is a surgical emergency, and a binder is used only as an interim treatment. I used 'urgent' vs 'immediate' to create a better contrast and because Amboss refers to it as 'urgent surgery

Amboss: UW also says binder are interim treatment
Interim management: Cover wound with moist dressing/Use adhesive tapes
Definitive treatment: urgent surgical revision
