Rationale for change

First of all, I added severe LV dilation as criteria per UW QID mentioned.

Additionally, I think that the wording here is confusing. Right now, it sounds like LVEF <=60% is the only indication, when in reality the indications are symptoms OR LVEF <=60% regardless of symptoms
