- Fasting serum gastrin means off PPIs
- Check gastric acid levels because failure of stomach acid secretion (pH > 4) may cause increased gastrin
- Gastrin level < 110 pg/mL rules out gastrinoma; values in between 110 - 1000 pg/mL require secretin stimulation testing
- Should be suspected in patients with multiple stomach ulcers and thickened gastric folds on endoscopy
