Rationale for change

Suggested text edit is (somewhat) unrelated, though I think more specific in boundary.

In the extra field, the 'Heart ausculation' image lists atrial septal defect as being both in the pulmonic as well as tricuspid area (which I believe to be an error), whereas the 'Heart murmurs' image in the same field shows atrial septal defect as being only in the pulmonic area (this is correct).

I believe it's an error, and that the 'Heart auscultation' image was meant to list 'ventricular septal defect', rather than atrial, at the tricuspid area.

Dunno how easy it is to fix, but thought I'd note it here.
