- Sickle cell disease can cause clots in the medial femoral circumflex artery (MCFA)
- This card recently was updated to include lupus in the mnemonic by adding an 'S' to CAST (Dec 2024)
- Other causes include vasculitis (e.g., lupus vasculitis), bisphosphonates, and denosumab
- Corticosteroids suppress osteoblast function and stimulate osteoclast function
- Sickle cell disease can cause clots in the medial femoral circumflex artery (MCFA)
- Trauma to femoral neck (e.g., fracture) can cut off blood supply
- Bends - when diving more gas is forced into liquid due to Henry's law, on slow ascent inert gas (nitrogen) comes out of solution via "offgassing" in the lungs; rapid ascent causes offgassing to occur in the blood, resulting in gas emboli that can affect the vasculature and tissues
- Legg-Calvé-Perthes - young children 5-10; initially painless limp but with gradual onset of hip pain (no knee pain) due to ligamentum teres femoris constricting before the MCFA has time to take over the blood supply
- SCFE - obese children 11-12; knee and hip pain; increased axial force on the femoral head results in the epiphysis displacing relative to the femoral neck
