Diagnosis of strep throat in {{c1::adults::population}} is based around the CENTOR criteria:

E: {{c1::tonsillar Exudates}} (+1)
N: {{c1::anterior cervical lymphadenopathy (Node)}} (+1)
T: {{c1::fever (Temperature)}} (+1)
OR: {{c1::< 15 years}} (+1) OR {{c1::> 44 years}} (-1)

- In children, CENTOR is unreliable; must get a (+) rapid or culture before starting treatment!
Interpretation:
Score ≤ 1: no further diagnostic testing needed
Score 2-3: Consider rapid strep test and/or throat culture.
Score ≥ 4: Consider empiric antibiotic therapy (controversial)
- A rapid antigen test cannot be used to distinguish between GAS infection and carriers. Thus, the judicious use of a rapid antigen test for patients with a score ≥ 2 prevents the unnecessary treatment of GAS carriers.
- Even with a CENTOR score ≥ 4, empiric antibiotics without confirmatory testing are not routinely recommended as the risk of acute rheumatic fever is not high enough compared to risk of inappropriate antibiotic therapy
