Exchange transfusion is indicated for:
- Total serum bilirubin value > 20 mg/dL in a 24-hour-old term baby
- Inadequate response to phototherapy
- Rapid rise in the total serum bilirubin level (> 1 mg/dL/hour in < 6 hours)

- Hemolytic disease with severe anemia
