Rationale for change

This card needs to be updated with the latest GINA 2023 protocol (see amboss). PRN SABA should no longer be used and formoterol+ICS (symbicort) is now the first line treatment. The way that I have constructed this card is messy, and I expect it to be rejected, but I hope that it can be used by someone else (one of the maintainers) as a template for an updated card.
