NOTE: Despite elevated serum potassium, total body potassium may be low
- Insulin is held if serum K+ reaches < 3.3
- When treated with insulin, K+ rapidly shifts from the extracellular space back into cells potentially resulting in a dangerously low serum K+ (hypokalemia)
- Insulin shifts K+ into the cells. Low or absent insulin in T1DM may lead to to high serum (extracellular) K+ and low intracellular/total body K+. This may be further exacerbated by the K+-H+ exchanger as it tries to compensate in an acidotic state (e.g., DKA)
