Rationale for change

Clarifying. I thought this was referring to "substance" coming out of a drain that was place during surgery, not an enterocutaneous fistula. I don't care how else it's worded but I think something should be added to clarify that it isn't draining from a drain but from the skin or incision site or whatever.
